1. Purpose of This Consent

This Document & Identity Verification Consent (DRAFT — attorney review required) explains what you agree to when you upload identity documents and capture verification selfies in the Elite Rides Driver app.

2. What You Provide

You may be asked to provide: • Images of government ID, driver's license, vehicle registration, insurance, and other compliance documents • A selfie (photograph of your face) taken with your device camera for identity verification • Related metadata (timestamps, upload status, review outcomes)

3. How Verification Works

• Uploads are stored in Firebase Cloud Storage with associated records in Cloud Firestore • Human reviewers/admins review documents and selfies and set statuses such as pending, verified, or failed • When a face-matching verification provider is configured, your selfie and reference images may be processed by that provider for identity matching • We collect identity verification photos for eligibility and safety. We do not claim Illinois BIPA biometric identifier collection/processing unless a configured face-matching program and counsel determine that BIPA (or similar laws) apply and required notices/consents are implemented • Selfies are not used for advertising

4. Your Promises

You represent that: • Documents are genuine and relate to you / your vehicle • Selfies depict you (not another person) and are not digitally substituted to deceive • You have the right to provide the materials

5. Retention and Sharing

Verification materials are retained as described in the Privacy Policy and Data Retention Schedule, and shared with service providers and reviewers only as needed for verification, safety, fraud prevention, and legal compliance.

6. Withdrawal

You may stop the process by not uploading materials or by requesting account deletion. Withdrawal may mean you cannot become or remain an eligible driver. Deletion timing limitations still apply.
